Once a month or more often if needed, our Care Liaison will come to your home to spend some quality time visiting and observing your care needs. During each visit time is taken to work on your personal care plan. Whether you are the primary care giver or an individual living alone this is an excellent way to maintain your independence while planning for necessary levels of support.
You may choose to have written updates sent to family members via email following each visit. These reports can provide a sense of peace to family who are too far away to see for themselves how you are doing.
